SRVUSD Volunteer System (for parent chaperones)
 
The 8th grade trip to Six Flags Discovery Kingdom is being held on June 4, 2024. 
 
Donations: 
  • $100 with a signed SRVUSD Permission/Health Form (covers transportation, park entrance and lunch)
  • $65 for students with a current Six Flags Season Pass with a signed SRVUSD Permission/Health Form (covers transportation and lunch). Students are required to bring their Six Flags Season Pass the day of the trip.
 
The permission slip/health form can be found on the CWMS website HERE and must be returned to Julie Mignano in the counseling office no later than ­Thursday, March 28, 2024.  Checks can be made to Charlotte Wood Middle School or you can purchase your student’s ticket from the CWMS Store HERE.  Please review the Code of Conduct with your student for expectations.
 
Parents and teachers will accompany students on each bus.  Students will arrive to school at regular time (8:15 am) and are expected to be back on their assigned bus no later than 2:45 pm for the return back to school.  The buses will leave the park at 3:00 pm. If any discipline problem occurs during the trip, parents will be called by an administrator to drive to the park and pick up their student.
 
This trip to Six Flags is a privilege.  Students who have not met promotion requirements are not eligible to attend.  The following criteria must be met before students may attend:
 
Behavioral Expectations:
We expect students to be on their best behavior between now and the promotion ceremony. Behavioral expectations for the activities are as follows:
  • One suspension: ineligible for Six Flags
  • Two suspensions: ineligible for Six Flags and the Party
  • Three or more: ineligible for all (Six Flags, Party, Ceremony)
 
Participation at all events per these guidelines is subject to the discretion of site administration.
 
We need parents to help chaperone.  If you are interesting in chaperoning this fun event, please visit the district volunteer page to either update your information or apply to be a volunteer (SRVUSD Volunteer System).  If you choose to chaperone, all the paperwork must be complete and filled out online with through the volunteer system.
 
Note:  No student will be denied participation for not being able to pay the requested donation amount.  However, if total donations do not cover the cost of the trip, we will be unable to run the trip. 
 
Return Permission/Health form and any donation to Ms. Mignano in the Counseling Office by Thursday, March 28, 2024.
